New LIFE Programs Coming to Lancaster and Williamsport


Albright Care Services (ACS) is pleased to announce the creation of two new LIFE Centers in Lancaster, PA and Williamsport, PA. ACS will be constructing the two comprehensive health service facilities in 2007.

LIFE (Living Independently for Elders) is Pennsylvania’s version of the nationally recognized Program of All Inclusive Care for the Elderly (PACE). ACS has been designated by the Pennsylvania Department of Public Welfare as the provider of the LIFE program in Lancaster and Lycoming/Clinton Counties. The participants in the LIFE program are persons over the age of 60 who would qualify to receive their care in a nursing facility, but who would prefer instead to live at home. Each LIFE program is based at the LIFE center (with a health care clinic and adult day site), which is the hub of the LIFE program. Participants come to receive direct attention for their medical, rehabilitation, and psychosocial needs; meet with the members of the interdisciplinary LIFE care team; get help with personal care; and participate in social and recreational activities. The LIFE primary care physician and the interdisciplinary LIFE care team coordinate and case manage the familiar elements of our traditional health care system across the primary, acute, in-home, and long term care continuum so that LIFE participants and their caregivers receive the most appropriate care and support. The LIFE care team’s focus on preventative care results in fewer hospital and emergency room visits for participants which  means reduced demand on the traditional health care system and lower cost of health care for many LIFE participants. This comprehensive program emphasizes preventative care, which is generally more effective and less costly than fragmented traditional health care, thus responding to the demands of participants, families, healthcare providers, and the government programs that pay for the care.
